This research marks the second phase of a study focused on noncommunicable diseases, particularly diabetes. The initiative pairs American sponsors diagnosed with Type 1 or Type 2 diabetes with elders living with diabetes in the Philippines. Phase one of this project was launched in 2023, where Unbound successfully partnered with Chronic Care International (CCI) to develop a cost-effective model of patient-centered care. Objectives of Phase Two
In phase two, the study's primary objective is to evaluate the health outcomes of participants over a 12-month period. Participants will emerge as sponsors, providing encouragement and support to their matched elders. This unique peer mentorship model will be evaluated by UNC researchers who will track changes in health outcomes, emotional well-being, and overall quality of life.
What Participants Can Expect
U.S. sponsors will be asked to contribute $40 per month, which will support their sponsored elder in meeting essential needs, such as medical care and medications. Each sponsor will also engage in at least three 30-minute surveys during the study to provide feedback on their experiences.

The elderly participants in the Philippines will receive numerous benefits through this initiative. Financial support will be provided in the form of cash transfers deposited into their bank accounts.
