how does insurance approve and pay for a drug that costs $238k a year when they routinely reject claims for common illnesses that cost nowhere near as much? 50%-80% claims rejected? wow

sounds extreme but lets just say the rejection is far lower, but still way too high, like 10-15%...how does a company like merck navigate this fact that insanely high drug prices may be motivation for insurance cos to deny the claim?
i just dont get it
and how do they settle on such a steep price? development costs? is leronlimabs shelf price going to go up if this development calendar keeps going the way it has? would like to see the math that is considered when deciding the price of leronlimab.
obviously this is not an issue for bargain basement leronilimab, if the day ever comes that i can file an insurance claim for it.
