As I recall Sullivan submitted the first form without seeking Pro hac vice admission. Once Rosen pointed out the error Sullivan filed for the correct standing. The judge won't be buying that B.S about unlicensed practice of law.

"Since Louisiana does not provide reciprocity for out-of-state attorneys, there are only two ways that out-of-state attorneys can practice in Louisiana. LA. SUP. CT. R. XVII § 11. Out- of-state attorneys must either seek admission to the Louisiana Bar or through pro hac vice admission.
