Thanks for the response. I certainly would like to find that form in some public domain also, as opposed to just accepting that MF has it and we can't find it anywhere else.

I'm not opposed to Bill selling his shares. He has every right to do so, but if the Table II information is factual (showing records of previous sale dates/quantities/values), it would be a bit concerning that there appears to be a pattern of information being released and him selling.
