just thinking out loud. if you have a filter, the BBB and it has certain size holes to only permit things small through, but then it becomes inflamed as a result of some inflammatory process, say the development of scar tissue or amyloid plaques or fibrotic overlay, then the holes would tend to get clogged even further and instead of getting bigger from swelling, their walls would grow closer together like a nose full of mucous not permitting any air through.
