I am not clear on what you are saying, but this is how vaccine trials are completed. The number of patients who get sick does not need to be in the tens of thousands to demonstrate efficacy.
Even if only 95 people are infected, the statistical probably that 90 were on placebo and 5 were vaccinated is extraordinarily low. (meaning clearly effective)
