Good work. Your numbers are similar to mine (I am using a two-group binomial distribution).
My two-tailed p-values for 24/21 (deaths LL/Placebo) is 0.0395.
For 21/24 p-value is: 0.002258
Regardless of the methodology used we are close to the number of patients we can "afford" for statistical significance.
