It would seem that these inflammatory pathways are inter-related and complex. A close focus on hyaluronic acid and edema seems to substantially ignore the coagulopathy that results from Covid and immune dysregulation(witness the autopsies with diffuse pulmonary thrombosis, profound enough to consistently induce remodeling of pulmonary vasculature via angiogenesis).
Many of the accounts of Covid ARDS indicate that it does not match classical ARDS. The V-Q mismatch of pulmonary thromboembolism, beyond just epithelial injury and edema, seems to have been unexpected at the outset of Covid.
Both Bradykinin production and coagulopathy can result from cytokine storm. The challenge, of course, is that the pathways are complex with multiple positive feedback loops.
Going WAY out on the limb, it may not matter tremendously what starts these pathways to dysfunction, but more what serves as a mechanism to interrupt these pathways (like school security wading into a brawl. Who started it? Who cares!).
I continuously return to the May 5 Patterson manuscript, in which he demonstrated not only reversal of T cellular exhaustion, repolarization of macrophages, reversal of cytokine storm and reduction of viremia, BUT also the elimination of ~4,000 RNA transcriptions which were the products of the deranged immune system.
Maybe regardless of how the story starts (Covid, Ebola, influenza), what matters is how is it finished.
