Yeah, it does seem a little fishy. I went ahead and sent a screenshot to see what forms will come back. Even if the screenshot were to be doctored, I'm sure the forms you have to fill out should be verifiable if this is legitimate.

I plan on showing them to Scottrade as well before filling anything out.
