New study reveals quality improvement opportunities for opioid treatment programs

A recent study published in JAMA Network Open, conducted by experts at RTI International, showcases the difficulties many opioid treatment programs (OTPs) face in maintaining patient engagement in their treatment. A focus on improving retention rates could significantly enhance recovery outcomes for individuals struggling with opioid use disorder.

The findings indicate that, on average, about 61% of Medicaid patients continue their treatment beyond the first month at typical OTPs. Alarmingly, in the lowest performing quartile of these programs, only 40% managed to keep patients engaged for a minimum of one month. This highlights a critical area for potential enhancement.

According to Tami Mark, Ph.D., a leading researcher at RTI, "Treatment retention is crucial for recovery and survival in patients dealing with opioid use disorder. This benchmark enables OTPs to measure their retention rates against national averages, allowing for targeted improvements." Such measures are essential in creating a path toward better patient outcomes.

Over recent years, researchers at RTI have utilized Medicaid claims data from 2018 through 2023 to develop case-mix adjusted retention metrics. In 2023, their analysis incorporated data from more than 1,100 OTPs, including 432,000 treatment episodes involving around 261,000 patients.

Variations in Retention Rates Across Programs

The data illustrates noticeable discrepancies in retention rates among OTPs:

At 30 days, the top performing OTPs maintained a retention rate of 73.5%, in stark contrast to just 40.9% from the programs in the bottom quartile. When extended to 90 days, the top retention rate was 54.8%, compared to 22.2% for the lowest performing programs. By 180 days, the results showed that 40.5% of patients remained in treatment at the higher performing OTPs, while only 11.4% were retained in the lower tier.

The variation in retention rates points to significant opportunities for quality improvement withinOTP settings. Identifying specific strengths and weaknesses can empower programs to implement effective strategies aimed at boosting patient retention, ultimately leading to improved health outcomes.
