AUTHORIZATION

Dear Applicant:

Your Notice of Intent (NOI) has been processed by Arizona Department of Environmental Quality (ADEQ). The referenced facility is authorized to discharge stormwater associated with industrial activities under the terms and conditions of ADEQ's AZPDES STORMWATER MULTI-SECTOR GENERAL PERMIT(s), MSGP.

Preparing and implementing a Stormwater Pollution Prevention Plan (SWPPP) tailored to your specific site is one of these requirements. You must be able to demonstrate compliance with all terms and conditions of the SWPPP as well as the MSGP(s) to avoid possible penalties.

Please keep this document for your records and use this authorization number for any correspondance. Reference this authorization number on the Notice of Termination (NOT) if the facility no longer meets eligibility requirements and must terminate MSGP coverage.
