Classic history. Classic exam. Classic tennis elbow.
Except the tendon was fine.
A patient came in with chronic lateral elbow pain. Every sign pointed to the common extensor tendon.
Five minutes with the probe told a different story: a tear in the radial collateral ligament, just beneath the tendon and closer to the joint. Scanning the asymptomatic elbow for comparison made it obvious.
At the lateral elbow, these two structures sit millimeters apart. Palpation can't tell you which layer is torn. A blind injection can't find it either.
What we did:
→ Talked through microfat, BMAC (stem cells), and high-dose PRP. The patient chose PRP.
→ 120 cc whole blood → 3 cc PRP → ~11 billion platelets. Know thy dose: that's about 3x the 3.5 billion I target for soft tissue.
→ Delivered under real-time ultrasound guidance, straight into the RCL tear.
→ Diagnosis and treatment at the first visit.
The lesson isn't about PRP. It's about the question that starts every orthobiologic decision: what are you actually treating?
The right biologic in the wrong structure is still the wrong treatment.
This is what we work through at the Regenerative Medicine & MSK Ultrasound Course, which I co-direct with Ben DuBois, MD:
???? Bellagio, Las Vegas · January 14–16, 2027
???? Friday: 4 hours hands-on scanning the shoulder, elbow, wrist and hand on live models, with faculty at your station
???? Bring your hard cases. We'll talk through decision-making, biologic choice and dose, and guided delivery.
Since 2008, more than 2,500 clinicians have trained with us. This year there are 50 seats.
$2,699 · $1,999 for active duty military, residents and fellows.
