My thoughts on yesterday’s CytoDyn presentation:
I thought it was one of the company’s more encouraging and informative presentations. It strengthened the case for leronlimab, although the results still need to be confirmed with mature clinical data.
The most impressive update was the ctDNA data. Among 28 evaluable CLOVER patients, the median decline was approximately 86%, with a roughly 90% decline at the 700 mg dose compared with 80% at 350 mg. The apparent dose response makes the biological signal more compelling.
This is also a very sick, heavily pretreated patient population. CytoDyn reported that 52% of CLOVER patients had received more than three previous lines of therapy, compared with only 13% in the SUNLIGHT trial. If leronlimab produces a strong response rate in these patients, that should get the attention of the FDA and potential pharmaceutical partners.
Management appears to be targeting an overall response rate of approximately 16.1% or better. That would be significant compared with the 6.1% response rate reported in SUNLIGHT and the approximately 2.7% rate seen in some real-world analyses of Lonsurf plus bevacizumab.
Other encouraging points included:
• No safety concerns after three Data Safety Monitoring Board reviews.
• Approximately 1,700 patients have now received leronlimab across its development history.
• The first patients in the leronlimab-plus-Keytruda salvage arm are expected to be treated soon.
• CytoDyn plans to discuss Breakthrough Therapy Designation, Fast Track or another accelerated pathway with the FDA.
• Management is preparing for pharmaceutical partnering meetings and believes the recent $16.5 million financing gives the company a stronger negotiating position.
The main caution is that ctDNA is still a biomarker—not final proof of clinical benefit. The FDA and potential partners will want to see confirmed tumor-response rates, duration of response, progression-free survival and, eventually, overall survival. They will also want convincing evidence that leronlimab is adding benefit beyond Lonsurf and bevacizumab alone.
My bottom line: The presentation increased my confidence that CLOVER may be showing a real biological and clinical signal. However, the most important confirmation should come from the additional clinical data at ESMO and the more mature update at ASCO GI in January 2027.
If the ctDNA declines translate into a confirmed response rate near or above 16%, along with durable disease control and continued safety, leronlimab could move from being an interesting experimental treatment to a legitimate regulatory and pharmaceutical-partnership opportunity.
