This is badly stated and does nothing to explain the mechanism of action of leronlimab. The immune checkpoint inhibitors are doing exactly what they're supposed to do, block what little PD-L1that exists. The PD-L1 is low because the immune system is not competent. An incompetent immune system will do very little even if all PD-L1 is blocked. Leronlimab restores the competency of the immune system by increasing CD4+ T-cells, CD8+ T-cells, natural killer T-cells and making them very active. The rise in PD-L1 is a response to that very active immune state because PD-L1/PD-1's purpose is to protect cells including tumor cells from destruction in an overactive immune state. Leronlimab's activation of immune cells that can kill tumors and its ability to downregulate other immune checkpoint inhibitors combined with a PD-L1 inhibitor is what makes it effective.
