I'll try to simplify it a bit.
Leronlimab and HIV reservoirs
Leronlimab in the long run downregulates IFNy, TNF-a and NF-kb ( proteins that are responsible for increasing inflammatory cytokines). That downregulation seems to be associated with a large increase in CD4+ cells. However for a few hours after the initial binding of leronlimab there would be a significant increase in them all. IFNy increases TNF-a which in turn increases NF-kb. NF-kb is the important one here. One of the things that NF-kb does is induce DNA/RNA replication including that of the HIV virus in viral reservoirs which then clears the reservoirs. It's not 100% efficient but it will clear many of them.
Leronlimab and the increase in PD-L1
Leronlimab by increasing IFNy temporarily activates the IFNy/JAK/STAT pathway which activates PD-L1 expression. Same IFNy starting point as the clearance of HIV reservoirs but different pathways.
