I think the ideal combination will include TAF because of its ability to be converted to the active ingredient inside the cells, viread, Which is considered a cornerstone of ART And an integrase inhibitor - be that dolutegravir (ViiV) or elvitegravir (Giliead), ralegravir (Merck) or biktegravir (Gilead). DOL and BIK are arguably the best choices.
But the obvious choice is to use Biktarvy as the ART Because it is the most widely prescribed ART That is a one pill once a day combination of TAF, viread, and biktegravir.
To throw a monkey wrench in things, ViiV Also has an entry inhibitor (fostemsavir) that they got approval for in the last several years. It blocks gp-120 Which is a co-receptor for HIV to enterT-cells along with CCR5. HIV binds to both receptors in order to enter the T-Cell. It’s used mainlt for HTE’s (highly treatment experienced ) patients and the drug/drug interactions along with side effects Make LL the obvious choice but I thought I should put that out there That there are other entry inhibitors- fuzeon (obsolete) etc.
