They must take that amount of levothroxine that keeps their TSH levels, T3 and T4 levels at homeostasis.
T3 is the REAL deal, it is the hormone and it is thousands of times stronger than resmetirom. you don't need nearly as much T3 to do the same work as 1000x more of resmetirom.
A patient cannot take levothyroxine in excess so as to treat NASH. It becomes intolerable and toxic as you've said.
Resmetirom can't give you a goiter. There is no iodine in resmetirom.
Resmetirom primarily works in the liver and its effects are not that systemic. It is a partial THR-Beta agonist. These -beta are in the liver. I believe the -alpha are in the rest of the body and those are not affected by resmetirom.
Resmetirom works to reduce intra-hepatic triglycerides. Reduction of steatosis. Main side effect is hepatotoxicity. Liver Function Tests need to be monitored during treatment.
