I know a cardiologist that finished a trans thoracic echo (TEE)…the imaging is fantastic because you are right behind the heart with the probe passed down the esophagus, unlike a conventional cardiac echo where you might be trying to get ultrasound images through 4” of fat on your BMI super-heroes. On a TEE you can actually see vegetative growth on heart valves. This patient had vegetation on the valve leaflets and once present they can eat through valve leaflets pretty quickly. They need to be on a broad spectrum aminoglycoside like gentamicin stat and full court press to action. On finishing the procedure the cardiologist went straight to the phone…sending you a patient for valve replacement, they should perform surgery asap. They waited the weekend, what’s the rush? Patient died of Disseminated intravascular coagulation. No option to put out the fire at that point…this young soul went to the morgue instead.
In the midst of a pandemic where there are refrigerated semi trucks parked outside the hospital to receive the COVID19 deceased because the morgue is full you don’t wait till Monday. There’s the quick and the dead…did they cover that with Jeff Murray in Medical School?
