Doc is playing a careful game of poker here and has to be careful not to expose his hand to those that have too many chips.

There was, however, no standardized form for physicians and other health care providers to report health care services. Therefore, the American Medical Association (AMA) embraced an assignment in the 1980s to work with the Centers for Medicare & Medicaid Services (CMS; formerly known as HCFA), and many other payer organizations through a group called the Uniform Claim Form Task Force to standardize and promote the use of a universal health claim form. As a result of this joint effort, the 1500 Claim Form is accepted nationwide by most insurance entities as the standard claim form/attending physician statement for submission of medical claims.

Even at (for example 10 cents) per transaction, each levy paid out when 50k+ pharmacies fill out the universal claim form (IFAH) someone pays the toll booth operator.
I thought I would mention this as it could be that simple.
