Transmission is going to happen in healthy hosts before there's much of an opportunity for immune response.. once that begins, the selection begins, and in healthy hosts titer drops below transmission threshold right around the time mutations of that sort would accumulate.
In asymptomatic patients there would be a limited time for mutations. In symptomatic patients mutations will occur and those mutations that initially escape destruction by the immune system will thrive while the original virus is killed off.
