All very valid points. the MOA is based on RO, high serum levels is not the whole story, as you have pointed out RO can vary from patient to patient. Yes the curves average to peak in 72 hours, my point is not degradation curves but based on bioavailabilty in the sub q space.
In a critically ill patient it is sponge full of water that certainly delays absorption to reach meaningful serum levels.
I have a few colleagues that support this point of view. I hope the study will look at this and develop a precision medicine based dosing curve.
