Everybody that drinks at all..knows what "Top Shelf" means...WE are top-shelf..described as "best of breed"..many times..remember that's a field of PEERS...others similar to us.
For the most part @ this point in time ....WE are STILL on the shelf....WE will become the Popular CHOICE for DOCTORS..that's ALL that MATTERS..
