Medicare info on Chronic Care Management. I believe there is a lookup tool for the code reimbursement towards the bottom. I know this is more info than most would want to see but I provide it in case you are interested. CCM is widely underutilized in the medical field. These codes reimburse providers for work they are generally doing anyway but never got reimbursed for in the past so it is new revenue. There is a ton of work done behind the scenes these days taking care of patients beyond the actual face to face visit. These codes pay us a monthly fee on about 60% of our patients. The software company that helps do this gets a chunk as will NuGenHealth in the Arizona market.

BTW, most all insurance companies cover these codes now.
Edit: I bet the AZ population of CCM eligibility is closer to 70-80% due to age and multiple chronic conditions in the elderly etc.
