A brief history lesson: Historically, given their origins as barbers whose state-of-the-art care consisted of blood-letting, dosing people with mercury and other toxic or repugnant substances, and drilling holes in the skull to "let the demons out," medical personnel (often untrained) were not addressed as "Doctor." That title was reserved for people who had earned Ph.D.s in academic disciplines.
As recently as the first half of the 20th century, in England physicians were referred to as "Mr." rather than "Dr." as their honorific.
Those who saw the now-deleted ravings of a sick mind will understand why I posted this information. Those who were fortunate enough not to be confronted with disturbing evidence of mental illness will, I hope, find this to be an interesting historical fact.
