Not trying to be arrogant, but in the course of my career I served on PT boards in my hospital and participated on several speakers boards for "expensive" never immediate action but eventually successful. The key will be compliance. My ID friend said most clinicians don't see pts on maintenance in their offices for PCR because compliance is horrible, they stop taking drugs, continue to participate in high risk behavior and don't show up for appointments, partly due to embarrassment, and obviously continuing transferring the disease. These would be the hidden costs for comparison other than the unit cost of the medication.
