My guess is that reducing plasma viral load in critical cases is likely to be clinically significant and improve odds of survival. We don't know if the effect extends to viral load in the respiratory tract tissues, but even so, keeping the viral counts low in the blood might well help prevent cases from progressing from mild/moderate to critical. Also, in serious cases, there are often terrible clotting problems, and eliminating viremia would help isolate the battle to the respiratory tract tissues. [these are all guesses by a non-MD, non-virologist.]
