Lets keep this off the boards. There is no point in calling people out. They might have added to the confusion BUT the main problem was the agency we used for Medicare approval process had a couple of other errors in the paperwork. It will be fixed and sent back in a matter of days. Believe me the agency will haul A$$ and applications are being expedited due to coronavirus. Here is another screenshot for better context:
