This is what happens when a company buys a very old airliner from a private operator - and tries to turn it into a tanker - rather than buy ex-military purpose built tankers or purpose modified tankers previously operated and maintained by a first rate air force [the $TMPS Tristars are ex-Royal Air Force].
In this instance, the Omega maintenance organisation failed to ensure the aircraft they bought had actually had mandatory safety modifications carried out.
