In accordance with Executive Order 202.10 issued by Governor Cuomo on March 23, 2020, your
laboratory has been APPROVED to perform TESTING RESTRICTED TO THE
DETECTION OF SARS-COV-2 IN SPECIMENS COLLECTED IN NEW YORK FROM
INDIVIDUALS SUSPECTED OF SUFFERING FROM A COVID-19 INFECTION.
This approval will be in effect until April 22, 2020, or until such date as determined in a
subsequent Executive Order. After such time, a clinical laboratory permit will be required to
perform testing on specimens originating from New York, in accordance with section 574 of the
New York State Public Health Law. Instructions and application forms for a clinical laboratory
permit are available at: https://www.wadsworth.org/regulatory/clep.
A copy of this approval letter must be maintained and may be subject to audit or review.
