Good point. Also, when you are a participant and you do your daily (or whatever the timeframe is) check with your trial doctors/nurses they ask you about everything that went on with you and your body since the last meeting. It’s not abnormal to have things like headaches or diarrhea in every day life but you still need to “report” it to the Investigators and they still need to list it in the trial results. (I believe that’s how Viagra came about, with an initial exploration of the molecule as a blood pressure drug and early reported side effects of, well, you know the rest of the story.) As long as the numbers aren’t abnormally high, the severity significant, or people stop the trial because of the side effect they’re all pretty much considered minimal.
