You are right , early treatment is very , very important , before damage is done..
Doctors in ICU observed that about 25% of patients there have heart muscle damage, cardiac enzymes are elevated in those patients ...
They not sure why , but they have at least 2 theories..
-- 1,) Patients have tachycardia , heart is overworking ,so blood supply is limited for the work.. this is call, ..." demand ischemia " ... ( not enough blood going into heart muscle for the work heart is doing.)
