I’m glad the trial w/ chloroquine and azithromycin is proceeding, although I personally would be wary of this combination. Yes, they both have anti-inflammatory effects and anti-viral activity; however, I would be very worried about toxicity. Chloroquine has a very narrow therapeutic range. More troublesome is that they both significantly lengthen the QT interval. With this population being mostly older and with often significant CV comorbidities, arrythmias are common. Not to speak of the arrythmias that are going to be a sequelae to the hypoxia caused by the ARDS itself. A well-constructed trial will hopefully disprove my concerns and evidence safety and efficacy.
But, again, this is why the trial is so important with Leronlimab which as far as has been shown over the years has none of these SAEs. Hopefully, the MOAs of re-polarizing the macrophages and suppressing Tregs will also prove efficacious in stopping the cytokine storm associated with ARDS.
