4kids....I agree with you that the date is definitely September 4, 2019. I have filled so many online forms which required to be either checked or put a number in the box but the form does not allow you to put the X or number in the box so you have to either put it before or after the box. the only box that Leo did not complete was the time of day which he probably may not have known at the time of filing.
