I believe almost all the delay with combo has been FDA issues. They have been pushing as fast as the FDA allows, but the FDA keeps changing their mind. Look at just the safety data, it was what 100 at 350mg, then they switched to 300 at 350mg in October 2017, then they switched to 150 at 350mg in July 2018, then then switched to 100 at 700mg in Feb 2019. Look at enrollment, it was what 100, then 30, then when they enrolled the 30 in June 2017, they switched to 50. I don't believe having a HIV specialist would have changed much there.
For example, look at RP with this TNBC trial. They thought they could get it up and running what in 1 month. But it looks like they have been negotiating with the FDA about the trial design. So even with someone with his background has to deal with the slow progress of government work.
My wife works for the VA, I am very familiar with just how slow the government can be. For example her latest job just applied for the job in November 2016, she interviewed in March 2017, she was offered the job in July 2017 and it took until February 2018 until she started her new job. Then she had two months of orientation to complete before she started the job officially in April 2018. It took 1.5 years! During that 1.5 years, there was no one doing the job, so it took almost a full year to get caught up. Basically 2.5 years later... Just crazy! That is what they are dealing with.
Now why does it look like everyone else is making progress? Like I say it is always 5 o'clock somewhere. If you have 200 drug companies, there will always be one with some result that just came out. Just like my daughter likes to tell me when all her friends are on vacation. I remind her, we vacation just as much as they do. It just seems that way since she has a lot of friends.
