I remember that prostate biopsies are really problematic and still not 100% accurate , so I did some reading..
Transrectal ultrasound guided biopsy -TRUS
It is actually " blind " and random procedure.
Some tumors especially the very small ones cannot be identified because ultrasound cannot visually detect specific cancer sites.
In addition TRUS biopsies tend to target the peripheral aspects of the gland and are likely to miss 30-40% of prostate cancers located in the anterior midline transition zone or apex...
It is called " sampling error "
To attempt to overcome this , they doing usually 12-14 samples , but sometimes as many as 24 called
"saturation " biopsies .
And the more samples the more possibilities of complication , and tumor still can be missed.
Right now little better results they see when MRI is use.
So , one sample needed for present Dr Pestell test is superior , plus it is prognostic test , so it is helpful with decision what type of treatment particular patient will need.
