Kaiser Health expose: FDA keeps secret registry of medical device failures, so doctors unknowingly keep using medical devices that kill or seriously injure patients despite FDA's awareness of problems. This is long but worth at least skimming. It's utterly horrifying and it's good that Kaiser Health is trying to bring this outrage to public attention:
