SJ- since I cant sleep I just read your post on IHUB about off-label use of drugs and insurance .
When doctor is using medicine "off-label " mean that medicine is approved for a different disease than doctor is prescribing , doctor can do it , but insurance dont have to pay for it , and this often is a problem...
Even for cancers , if it is approved for one , they don't have to cover others.
But when we talking that after pivotal is finished for mono , and combo is approved by then , company can ask FDA for " Label Extension ", and if FDA approve that , insurance need to honor that as normally they doing with every other approved medicine...
