After researching the subject I realized that if the goal of the trial is to see how many patients will show improvement of at least 70% the actually efficacy results could be much higher or lower depending on how the creme works on each individually. efficacy is "the extent to which a drug has the ability to bring about its intended effect under ideal circumstances, such as in a randomised clinical trial" Correct me if I am wrong.
