However in the 50's, 60's and 70's a doctors visit was $5-10 and a script if needed another $10. There was little need for insurance unless you needed hospital care. Before that we are really getting into the words "practicing medicine ".
Today we have $10-50 co-pays for both the doctor and script that cover overhead and the rest is insurance company or HMO profits from premiums. Anything that eats into the insurance premium is considered a negative and revenue loss.
Life style choices seem to be the niche these days for increased premiums. However genetics or DNA is a bigger factor.
