as a Radiology Manager my overhead cost on all examines was $18.00 per exam.
The charges to the insurances was $100- $2000 per exam with Doctor cost being 50% of that so $50-$1000.. so Total cost to insurance was $150-$3000 per exam

Medicaid and Medicare paid at 30% or so. So you have $45 - $900 paid $18 cost to Hospital and lets say for the fun of it $18 for the doctor...so cost $36
Insurance 80/20 for most after you meet the deductible $104 -$1000 per month premium...
