Mini 1- Had a pressure issue in the emergency sequence that Baltia took from Northwest since that was where N706BL came from. This pressure difference in the plane from the outside made it hard to make the doors open easily. Baltia did not deploy slides this day, they changed their emergency procedures to resolve this problem and preformed Mini 2 the next day.
Mini 2- Crews performed Mini 2 flawlessly but 5R and 2L slides failed to deploy correctly. It was believed it was a issues with the doors and they were adjusted.
Mini 3- Crews performed flawlessly but the FAA controller of the Door Blocks delayed activation of block indicator on one door that was to be blocked which resulted in 6 doors opening. Door L5 failed to deploy but 5 slides did deploy making it meet the FAA requirement of 50% of slides deployed. Rumor has it 30 of 31 FAA inspectors wanted to give them a pass but ruled on the one no!
Mini 4- Crews performed flawlessly but R1 and R4 didn't deploy correctly. It was believe to be a door issue still and sent plane to Oscoda to check doors. No issue found with doors there by Kalitta.
Mini 5- Crews performed flawlessly but R1 didn't deploy. I believe after this test is when they found the issue with the strap being to short on the slide packs preventing the trigger strap to extend fully to deploy the slide. This find is now thought to be the issue the whole time and the industry wide problem of why slides fail to deploy.
Mini 6- Crew performed flawlessly once again but R5 Girt Bar on slide failed. All slides deployed though! It was not said at the meeting but I have heard that fowl play may have been involved with that and someone was fired in maintenance.
Mini 7- Crew performed flawlessly and set the record of I think they said 8 secs.. All slides deployed but slide R1 tangled in the middle stage of unfolding due to the guide ropes entangling twisting the slide preventing it from touching the ground. They show the picture of the twisted slide.
After Mini 7 all slides were taken to the repacking facility and repacked with Baltia observing every step with manufactures Manuel of how 747-200 slides were to be packed. Found that the repacking facility was not using the manual and had check lists that were generic steps and not packing correctly in previous slides.
This is all from memory at the meeting and looking back at SDRs. I may have some Mini's crossed but this is my best from what I remember!
