Which adds to my point, if they are actually electronically filing and signing these things themselves, then it is totally intentional and an obvious scheme.  But it seems more so that someone has been delegated the duty's for tracking and filing these requirements and isn't taking the time to proof read and or thoroughly check the forms................................................................At least that is what I chose to believe.
