Florida Hospital will employ the InscrybeMD telehealth solution to help its patients with the follow-up care they need to manage chronic diseases and conditions, including Chronic Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and Diabetes and Hypertension, through Florida Hospital Community Care, a recently piloted partnership between the hospital and Bethune-Cookman University. Through this program and the implementation of the InscrybeMD telehealth solution, the hospital aims to improve a patient's health status, reduce unnecessary ER visits and hospital admissions, and improve patient satisfaction and experience. The care coordination is performed within the patient's home, overseen by an interdisciplinary clinical team, and supported by student health coaches from Bethune-Cookman University.
