If I were you, I would immediately either call Matt or send him an e-mail with your name, # shares owned, email address and name of your broker. He should referrence the information to Broadridge, who is responsible for sending the ballot. Please do so ASAP.
Anyone who hasn't received his/her ballot yet, should do the same.
You can get Matt's e-mail from any pess release.
