What we know of ivermectin - large properly done trials show no effect. What we know of leronlimab - Even in a trial that was skewed and was underdosed it showed effect in a subpopulation. What we know from the very first EINDs - In a population with dire comorbidities that was on death's door and the drug was used only because death was imminent some patients recovered. That recovery was backed up by biomarker data. What I personally know - the pathways and downward cascades that are effected by leronlimab and implicated in Covid-19.
What do we know about ivermectin? The dose necessary to inhibit viral reproduction is 35 times higher than the recommended dose. The dose necessary for significant reduction of cytokines is 4 times the recommended dose and this was only studied in lipopolysaccharide and toll like receptor 4 induced inflammation. It may not hold true for other inducers of inflammation.
