I couldn't cast my vote either. E*TRADE did not send any proxy card/number.
Called the provided telephone to vote and was told that, even being in the list of shareholders, I needed the 6 digit number. Otherwise no way Jose.
Not sure what the issue is/was but I usually receive these cards promptly.
