Can't you still get it through the Open Label Extension (OLE) for Severe or Critical?
It's still on their website. There are 18 sites per my count. I believe these are the same sites at the CD12.
Problem is that by the time you are in need, you may not be in a condition to demand to be taken there as you might be on a vent or breathing equipment. (Hard to demand something when you are unable to speak or in an induced coma). You really need a trusted relative or caregiver to demand that you be brought there and hope you qualify and they accept you. I have long ago make clear to my relatives as to which hospital to take me if I am ever in need.
