Full blood panel (cbc/bmp) recent and one a few months back, along with entire pt Hx would be helpful in determining a differential diagnosis. Ohm would be the most knowledgeable as far as past Leronlimab/pro140 studies indicating any hepatic (enzymes) transaminases that were elevated. Never heard of any involvement (related to increases) whatsoever in any indication we've trialed.
Is hepatic carcinoma the primary tumor? Could be indicative of matastis to the bones. Without knowing how high ALP is, along the patients entire panel, it would be hard to say. For instance having a 2x-3x elevation of ALP, can have different dx than a 10x increase. Details are key.
You did say, pt had a "new" liver stent recently placed. These stents don't really last too long, less than 6 months for sure, depending on the material used.
My bet is early failure of the stent, leading to a blockage of the bile duct (again). I'd re-check bilirubin levels for sure. Regardless, that's the first place I'd look (it would be a mistake to stop LL, to rule out the cause IMO).
