NHIT - MEDICATION AND IMMUNIZATION MANAGEMENT INITIATIVE
The National Health IT Collaborative for the Underserved, Inc. (NHIT®), EagleForce and Health Gorilla have launched the NHIT - Medication and Immunization Management Initiative (MIMI Rx) to help patients and their caregivers manage their medications. Taking advantage of the perfect storm of genomics, precision medicine, constantly developing IT innovations and utilizing modern health technology based on APIs, this new initiative will, at the outset, help to solve the most distressing problems experienced after Hurricanes Irma and Maria in both Puerto Rico and the Virgin Islands - the inability to track patients in need and those who were evacuated, as well as to ensure access their medical records. This failure resulted in heartbreaking experiences for patients and their families and, for too many, in preventable loss of lives.
The Virgin Islands evacuated 807 patients - some with medical records pinned to their clothes, and many without any records at all. 55 are known to have died. We witnessed or heard so many heart wrenching stories.
The NHIT - Medication and Immunization Management Initiative is NHIT’s and the NHIT Care Campaign commitment to delivering on the promise of a resilient health safety net for years to come.

Medication can be a lot to manage, especially when taking multiple prescriptions with different sets of specific directions to follow.

❖ Of adults over the age of 50, 75%take prescription medication on a regular basis.
❖ Over 43% take 4 or more prescription medications on a regular basis.
Medical Information
Most adults may not have the ability to access their healthcare information. Without this vital data, medical professionals are left with
informational gaps, which may result in a patient not receiving the best possible treatment options.

❖ Poor communication of medical information causes up to 50% of all drug errors and up to 20% of adverse drug effects in the hospital.
❖ 27.5% of hospitalized patients received an inappropriate drug.
