They are saying that the actual infection and death toll in India could be 10 times what they are admitting.
This is an unimaginable catastrophe.
"First do no harm" needs to be understood in a new context:
"if they are going to die, give them the untested medicine in case it works -- especially if it has a good safety profile from other clinical trials."
